<h2>Q: Bulge Directly Under Eyelids After Lower Bleph. Residual Swelling?</h2>

<p>I had a lower bleph 2 1/2 weeks ago. As acute swelling died down, I noticed I now have direct bulges under my eyelid incisions. I know I am early in the recovery but is it possible to tell if this is swelling that will disappear in time or a part of my anatomy? Left bulge is more prominent &amp; soft to the touch,leads to believe its swelling. Would swelling stay this high in the eyes? Its irritating that I had this to remove one problem and am now left with these bulges. Something to worry about?</p>

<h2><span class="a">A:</span> Lower eyelid swelling can be quite persistent!</h2>
<div class="content-body">
<p>The bulging you have is quite normal and is related to the junction between the thin, easily stretched lower eyelid skin and the heavy cheek skin which does not swell as easily. The fluid will follow the path of least resistance and once it hits the cheek eyelid junction it can not easily flow further down and must be absorbed over time.  If the bulge were higher up, it could represent residual fat hernia which is most common in the lateral compartment of the eyelid.  In these photos, you should rest assured that things will settle relatively soon.</p>

<p>I am 40 and have genetic under-eye hollowness. It has become markedly worse over the past few years. I really want to do something more on the permanent side, but am now thoroughly confused: An oculoplastic doctor has said that I am not a candidate for a lower bleph/that it wouldn&#8217;t make me look good. I have, however, seen that it gives amazing results on plenty of people with similar appearances. So &#8211; do I add or remove? Advice welcome! Thank you.</p>

<h2><span class="a">A:</span> Fillers are great, but lower eyelid blepharoplasty has greater longevity.</h2>

<p>Hi,</p>
<p>Although I am a huge advocate of the use of Radiesse to reestablish the roundness of the cheeks and the fullness between the upper cheek and the lower eyelid which will have the wonderful benefit of blunting the fat hernia, I believe your best choice would be a blepharoplasty.  I do both procedures in all aged individuals.  In your case, I would recommend a lower eyelid blepharoplasty.  Your skin tone is excellent and your face appears very young. The Radiesse is great, especially as the face deflates and sags but your primary problem is the visibility of the fat hernia.  The lower eyelid blepharoplasty is a terrific and effective procedure with great longevity.  This will address the issue rather than mask it. The cost of the Radiesse over a short period of time will be greater than the blepharoplasty.  The healing from the blepharoplasty will be quick- sutures are out in 3-4 days and echymossis resolves within 2 weeks ( considering you have no complications!).  Good luck!</p>

<h2><span class="a">A:</span> Your eyes are not weird- they are very pretty and your deep creases will allow you to age more gracefully!</h2>
<div class="content-body">You have very nice and symmetric eyelid creases and you DO NOT HAVE PTOSIS!!!  This reference is related to the position of the eyelid relative to the color portion of the eyes and yours are normal.  The deep set position of the crease of your eyelid is a genetic finding. I am sure you have always had this, correct?  When you get older, this deeper crease will allow the excess skin to fall back and not show as much so you will not need upper eyelid surgery until later in life.  There are ways to try and change the position of this crease but typically with eyelid surgery, we place the suture line in the crease so it is not visible.  Your darker pigment may make a scar outside of the crease more visible.  If I were you, I would recommend you enjoy your beautiful face and take advantage of the larger crease for use of make up which will enhance your eyes &#8211; even very simple make up like a painted on eyeliner- this may make you feel better about how you look- Try visiting a cosmetologist and get their recommendations but really, I think you are being unreasonably hard on yourself regarding your eyes</div>

Would eye lid tape be helpful as a non-permanent option, or would it be too difficult to maneuver to look well done? I understand my eyebrow adds to the unevenness but I&#8217;m more concerned about the lid. Thanks</div>

<div class="content-body">Everyone has asymmetries and as stated- when you look at models etc. the most beautiful typically have more asymmetries than the less- so enjoy it!  It is a normal so please don&#8217;t try to do something to your face that you will regret- Your eyes are nice!! So move on and enjoy how beautiful they are while you are still young!!!</div>

<h2>Q: Is it okay if I&#8217;ve consumed alcohol four days previous to a blepharoplasty (double eyelid surgery)?</h2>
<div class="content-body">Today (July 6th) I&#8217;ve received a blood test. I have an appointment to undergo blepharoplasty on the 8th. However, on July 3rd, I&#8217;ve consumed alcohol and smoked a cigarette, and on July 4th, I&#8217;ve consumed three cans of beer. I&#8217;m worried because I&#8217;ve heard that alcohol and cigarettes can interfere with the safety of the procedure. I normally NEVER smoke or drink except those two days. Is it safe to proceed with the blepharoplasty? Also, would my cigarette and alcohol intake show in the blood test?</div>

<h2><span class="a">A:</span> Smoking increases your risk for poor healing and infection- alcohol affects the pain medication effectiveness</h2>
<div class="content-body">The smoking is the primary problem for your surgery.  If you are not a chronic smoker, the effects are going to be less damaging and because it was 4 days prior you will likely have had time for your body to recover significantly.  You should tell your doctor and be prepared for slightly increased risk to not heal as well or to develop an infection.  The eyelid skin is so thin and the blood supply there is so good, it is unlikely that you will have a problem, but if you do, only your actions are to blame.  Regarding the alcohol, it is really more of an issue to not drink if you are taking a narcotic. Mixing these can have serious risks to your body.  Also, if you drink consistently, the effectiveness of the narcotic may be reduced or altered.  This should not impact the surgery unless you drink consistently enough to have damage to the liver which affects the ability of the blood to clot.  That is a long term risk of alcohol intake, not a short term risk.  Discuss with your doctor and let the doctor have the final decision.</div>

I&#8217;m worried about using fillers and the long term cost of using them. Seems like surgery makes more since $ wise. Would this be a simple case or more complicated case on the spectrum of lid procedures</div>

<h2><span class="a">A:</span> Lower eyelid fullness is common at a young age! Blepharoplasty/ eyelid surgery is great!</h2>
<div class="content-body">It is not uncommon to have the lower eyelid fat bulging from childhood. This is a genetic predisposition which you may find several members of the family have the same problem.  The fillers done well will often mask this problem, however, the cost of the surgery will be surpassed in short order by using fillers and the risks of the fillers is present with each injection.  If you are tempted to do surgery, go for it!!! It is a long term solution with one recovery.  You will be VERY happy with the results and the fact that people will stop saying you look tired.  You only need to do the upper eyelids if you have excessive skin there.  If so, it is worth it is probably worth it to do them both and have one recovery. Your photos are inadequate to assess this so you will need to have a consult with the plastic surgeon to best evaluate this need. If you do have excess upper eyelid skin and you do the surgery, you will feel and immediate relief of the tired/aching eyes that commonly happens as the upper eyelid skin accumulates! Good luck!!</div>

<div class="content-body">I have droopy upper eyelids that are very uneven. One side has much more excess skin than the other. I&#8217;d really like to remove the droopiness altogether, or at least even them out, but I don&#8217;t like the idea of surgery. Are there any other options?</div>

<h2><span class="a">A:</span> Upper eyelid surgery is easy, effective and the only true solution!!</h2>
<div class="content-body">Surgery is the answer!!! Do not worry.  The plastic surgeon should be able to excise the skin in a very artistic manner and solve your problems- it is really like having a mole off with the added artistry to give you a wonderful result.  Your eyes will look like they did when you were younger and you will not feel the weight of the skin which will relieve your aching in the eyes at the end of the day.  The stitches come out in 3-4 days! You can have the surgery done with straight local anesthesia which is easily tolerated- only a little sting with the anesthesia for 3-10 seconds per side!  I would not trust a heat related option as you have less control and may have to deal with complications and asymmetries which surgery will control by careful measurements.  Just go for it!!! You will not regret it once it is done!  Be sure to choose a Plastic Surgeon who has good results by photos, and details the closure- look at photos so you can be sure you will get the results you are hoping for!!Good Luck!!</div>

<h2><span class="a">A:</span> Blepharoplasty will do!</h2>
<div class="content-body">Upper eyelid blepharoplasty will solve your concerns.  The upper eyelid skin excess is the problem which causes you to look tired and makes your eyes itchy or achy.  The brows do not need to be lifted, they are in a good position with proper amount of show of the lateral orbit. Lifting the brows will give you a surprised look.  The Botox can be used but it is difficult to consistently achieve a lift and also the cost adds up!! The upper eyelid blepharoplasty is only a few 1000 and will resolve your problem for years.  Also with removal of the excess skin not only will your eyes feel better, you may not need the botox to the forehead wrinkles as much because you will not be lifting your brows to relieve your eyes of the weight of the excess eyelid skin.  The eyelid skin excess is a genetic problem related to the size of the orbit and the quality of your skin, so age 26 is not an uncommon age for this procedure.</div>
